This position is an advanced practitioner of therapeutic services in a Permanent Supportive Housing program that offers both Housing First and Sober Living options. The Permanent Supportive Housing Therapist will serve Park Center members on a recovery-oriented, housing-focused intensive case management (ICM) team.
ESSENTIAL FUNCTIONS

Take deliberate steps to increase their own cultural competency by attending training, events, discussions, workshops, etc.Member Support Duties :Engage members in their homes and in office locations utilizing a trauma lens and cultural humility to quickly establish trust and build rapport.
Partner with members to help them achieve goals around improving their mental health and substance useProvide individual therapy sessions for members with mental health challenges, substance use disorders, and physical health issues. Use evidence-based modalities such as Cognitive Behavioral Therapy (CBT), Dialectical Behavior Therapy (DBT), Motivational Interviewing (MI), and harm reduction strategies.Facilitate group therapy sessions focusing on topics such as trauma recovery, mental health management, addiction recovery, and coping with physical health issues.Support PSH members by offering recovery focused services that address wellness of both mind and body.Educate members about the connection between mental health, substance use, and physical health, providing tools to enhance overall wellness.Empower members by teaching coping strategies, life skills, and relapse prevention skills.Collaborate with members to develop personalized treatment plans that address mental health, substance use, and physical health needs.Provide crisis intervention for members in acute distress, ensuring safety and immediate support.Address mental health and substance use crises with sensitivity and a trauma-informed approach, coordinating with medical and social services teams as necessary.Assist PSH members with developing interpersonal skills to connect with members of the community and expand their natural support system.Complete, maintain, and review documentation for agency and funding requirements within all designated timeframes.Lead recovery focused groups in PSH housing for members to attend.Develop and provide resources to members based around recovery services.Assist PSH members with developing interpersonal skills to connect with their housemates and neighbors, and to expand their natural support systemCreate, maintain, and review documentation for agency and funding requirements within all designated deadlines.Public / Staff Relations :Communicate with internal and external partners to coordinate care and advocate for member needs.
Communicate regularly with the Permanent Supportive Housing team to meet member needs.Build rapport and connections with community service providers to provide wrap around support and care to members in recovery.Develop effective working relationships with human services organizations.Program Development :Create and update departmental procedures in collaboration with PSH leadership.Coordinate and meet with other programs and industry leaders for research and development.Work closely with Recovery Support Coordinators, Program Managers and other service providers to ensure comprehensive, recovery focused care for members.Participate in multidisciplinary case conferences to discuss member progress, challenges, and additional needs.Other duties as member of the Permanent Supportive Housing team :Attend all staff and supervision meetings.
Assist with program events and outings.Maintain schedule flexibility in the event of after-hours responsibilities.Maintain good attendance and inform supervisor in advance of time off.Complete Relias Learning courses by deadline.Follow all Personnel Policies and uphold Park Center’s Code of Ethics.Perform all other duties assigned.SKILLS AND QUALIFICATIONS
Master’s degree in social work (MSW), Counseling, Marriage and Family Therapy (LMFT), or a related field required.Licensure in the state of Tennessee (LCSW, LMFT, LPCC, or similar) preferred, or working towards licensure.At least 2 years of experience providing direct clinical services[DLM1] , preferably with individuals experiencing homelessness, mental health issues, substance use, and / or physical health concerns.Experience working with trauma, substance use recovery, and / or co-occurring disorders is strongly preferred.Knowledge of harm reduction principles and practices is a plus.Able to demonstrate strong recovery support competencies and ethical behaviors.Must have proven written and verbal communication skills and computer literacy.Must have a valid Tennessee Driver’s License.Must be able to show proof of personal automobile insurance.WORKING CONDITIONS
