Your Mission (Should You Choose to Accept It):
- Provide direct patient care by administering medications and treatments as prescribed, ensuring our residents receive the highest quality care.
- Conduct thorough patient assessments and document medical histories accurately to help guide treatment decisions.
- Perform catheter care and venipuncture as needed, ensuring patient comfort and safety.
- Collaborate with interdisciplinary teams to create, implement, and adjust patient care plans.
- Implement infection control measures to maintain a safe environment for both patients and staff.
- Maintain accurate medical documentation in compliance with healthcare regulations, keeping the care process transparent and efficient.
- Offer support and education to patients and their families, providing clarity and guidance on health conditions and treatment plans.
- Monitor vital signs, reporting any changes to the nursing team to ensure prompt action.
- Work closely with geriatric patients, addressing their unique health needs with compassion and professionalism.
- Lead the way in implementing and maintaining skin care treatments, ensuring our residents' comfort and skin health.
- Collaborate with the Care Planning Team to develop personalized skin care treatment plans.
- Be the first responder to requests from residents, physicians, and nursing staff, stepping in when needed and offering support.
- Identify, manage, and treat various skin conditions like abrasions, ulcers, and infections, helping residents heal and thrive.
- Communicate findings clearly and compassionately with residents, families, and the medical team—because transparency and empathy are key.
- Prepare detailed reports and recommendations to the care team, ensuring our residents receive optimal care every step of the way.
What We’re Looking For:
- At least 1 year of experience as an RN/LVN in a hospital, long-term care, or other healthcare setting.
- An active, valid RN/LVN license for this state.
- Strong understanding of infection control practices and medical documentation standards to ensure patient and staff safety.
- Experience with geriatric patient care is an asset, as we specialize in caring for seniors with diverse needs.
- Excellent communication skills, both verbal and written, to effectively engage with patients, families, and healthcare team members.
- Ability to work collaboratively within a team while also demonstrating strong independent decision-making skills.
- Certified Wound Nurse training? A plus, but not required!
