As an **MDS Coordinator - LVN**, you will be at the heart of our mission, driving Medicaid and Medicare reimbursement efforts while ensuring compliance with federal and state regulations. Your focus will be on the coordination and completion of clinical assessments, supporting documentation, care planning, and the timely transmission of MDS 3.0. **Salary**: Starting at $76,000 annually

**Schedule**: Monday-Friday, 8 AM - 5 PM

**Key Responsibilities**

• -Coordinate and complete clinical assessments for Medicaid and Medicare reimbursement, ensuring all documentation supports MDS coding.
• -Create, review, and transmit MDS 3.0, ensuring compliance with federal and state regulations.
• -Lead the care planning process, ensuring comprehensive, individualized care plans are developed and aligned with resident needs. Monitor MDS and care plan documentation, ensuring it is organized, chronological, and consistently up to date for all residents.
• -Monitor and track reimbursement processes, ensuring accuracy and compliance with both Medicaid and Medicare.
• -Collaborate with interdisciplinary teams to provide education and guidance on MDS and care plan completion.
• -Ensure that all documentation is complete, accurate, and compliant with regulatory standards.
• -Schedule assessments for new admissions, annual, quarterly, Medicare, or change-of-condition reviews, while coordinating communication with nursing and other departments.
• -Coordinate and attend regular Medicaid meetings to discuss current residents, pending residents, and recipients.
• -Ensure timely transmission of MDS, meeting all deadlines and requirements.
• -Assist with ancillary management, data collection, and reporting to billing personnel.

**What We’re Looking For**

• -LVN License required (RN encouraged to apply), in good standing.
• -5 years of MDS experience in a geriatric healthcare setting.
• -Restorative care philosophy, excellent professional skill (both assessment and hands-on -and good communication and organization skills).
• -Experience with **Medicaid and Medicare** reimbursement processes.
• -Strong understanding of **long-term care regulations** and compliance standards.
• - A commitment to staying updated on industry changes through workshops and conferences.
