Work Schedule : 1.0 FTE - 8 : 00am - 4 : 30pm with occasional weekend / evening leader on call
POSITION SUMMARY :
To provide supervision and leadership for the Hospital Care Management team, including RN Care Managers, Social Work Care Managers, RN and Social Work Supervisors / Leads, support, and other affiliated staff in both the inpatient and Emergency Center settings. The manager will assure the successful development and implementation of care coordination strategies as they relate to the department. To act as a clinical and process consultant for the team and hospital regarding care coordination and transition / discharge planning. To actively support hospital care and system improvement initiatives with increased focus on identified areas of opportunity with hospital care management and affiliated activities.
MINIMUM QUALIFICATIONS :
- Education, Experience or Equivalent Combination :
Master's degree in Social Work and / OR Bachelor's / Master's Degree in Nursing
Licensed Independent Clinical Social Worker (LICSW) licensure and / OR Registered Nurse (RN)
Pager, vocera, computer, electronic medical record, fax, scanner
PREFERRED QUALIFICATIONS :
Master's Degree in Social Work or Nursing
Clinical Supervisor Certification through the Board of Social Work if LICSW
Demonstrated effective leadership and supervisory skills.
