Licensed Social Worker-Per Diem
We are seeking qualified Licensed Social Work Professionals to join our team in our Nursing Home. We want to speak with naturally caring, empathetic Social Workers who are focused on providing the best quality of life possible for every patient in their care with patience, courteousness and tactful interactions with residents, staff, family members and visitors.
We are seeking per diem Social Workers to work the following shifts : Weekdays from 5 : 00pm to 8 : 00pm, Weekends from 8 : 00am to 4 : 00pm.
Position Overview
Reporting to the Director of Social Work, Social Workers at Parker deliver quality social work services to assigned patients, residents, family members and significant others. The Social Worker is responsible for coordinating the discharge planning process, participating in interdisciplinary team meetings and quality improvement activities, as well as counseling and meeting the satisfaction of the patients, residents and family members.

Position Qualifications
The ideal candidate will have a Master's Degree in Social Work as well as active licensure in NYS, with prior experience in a long-term care, rehabilitation or hospital setting. Prior experience working with the elderly population preferred. Additionally, strong applicants will possess strong communication skills, and an ability to work collaboratively across a variety of disciplines and familiarity working with EMR (electronic medical records). Must be vaccinated against COVID-19.
