- Two (2) or more years of experience as a Physician in a hospital with a geriatric population; or within a nursing home environment
- A medical degree from an accredited medical school
- Board certification (or eligibility) in a primary care specialty
- Completion of residency in Internal Medicine
- Eligibility for hospital admitting privileges
- Clean and active license to practice medicine in New York State
- Proficient in SigmaCare or a similar Electronic Medical Record software
