Parker Jewish Institute for Health Care and Rehabilitation, located in New Hyde Park, NY, is seeking a Registered Nurse to join our Admissions team as a per diem Admissions Coordinator on the weekends.
As the Admissions Coordinator, you will play a key role by completing pre-admission assessments (PRI) in conjunction with the Admissions Assistant Vice President, to identify medical, social and financial issues with potential new admissions, and ensure that patient's needs will be met upon admission by collaborating with the Parker team. You will act as an ongoing patient liaison, serving as the initial contact for patients coming into Parker, while maintaining ongoing metrics and quality improvement initiatives.

Position Requirements
-
Current and valid Registered Nurse (RN) license in NYS

Experience in a hospital setting and/or skilled nursing preferred

Experience with admissions, discharge planning and case management

Knowledgeable in PRI and long-term care reimbursement issues

Ability to work independently and with a team across a variety of disciplines
- Availability to work weekends
Salary : $50 - $53
