The Parker Jewish Institute for Healthcare and Rehabilitation, located in New Hyde Park, NY, is seeking an RN-Home Care Admissions Nurse knowledgeable in admissions assessments, to join our team and play a key role at Parker's Certified Home Health Agency.
Parker Jewish Institute Hospice and Home Health Care offers the finest in home health and hospice services from an interdisciplinary team of health care professionals. Parker Jewish Institute for Health Care and Rehabilitation is a faith-based, not-for-profit health care system comprised of multiple health services servicing the Metropolitan New York Area, including Long Island, Brooklyn, Queens, Bronx, Manhattan and Westchester.
In this position, the Admissions RN is responsible for ensuring that Parker's programs and services are effectively represented to patients, families, and others who have an interest in home care through pre-assessments, information sharing and/or admissions in a patient's home. The Admissions RN works closely with the intake team, referring physician and other referral sources to promote the CHHA philosophy of care. Assessments may take place in private homes, hospitals or nursing facilities.

Position Qualifications
Current New York State RN license
BSN a plus
1-2 years of experience as an RN
Experience in a Home Care, Hospice or Community Health environment strongly preferred; experience as an intake RN a plus
Proficient in EMR software; familiarity with Allscripts a plus
Access to reliable transportation; ability to conduct home visits as needed
Vaccinated against COVID-19
