This position will ensure that the health and safety of all clients are met. Working cooperatively with the Administration and Management staff, supervise the delivery of services provided to the clients and supervises staff.
QUALIFICATIONS:
- Must have current and unrestricted LPN License with the State of Minnesota and must maintain throughout employment.
- Must have a valid driver's license.
- At least 4 years experience in related field.
OTHER SPECIALIZED KNOWLEDGE AND ABILITIES:
- Possess a sensitivity and aptitude for working with elderly and disabled residents..
- Possess skills to communicate effectively with those you supervise, clients, family members, vendors, community, management staff and Director.
- Possess leadership / supervisory skills.
- Must be able to prioritize and organize work effectively and efficiently.
- On Call weekend rotation
ESSENTIAL JOB FUNCTIONS AND TASKS:
Job Function: Manage Home Care program including completing essential paperwork and complying with state regulations.
Tasks:
- Works to assure facility is in compliance with federal, state and local standards and regulations. Will be available to participate in license surveys
- Keeps forms updated including policies, procedures, Bill of Rights, and related home care forms and manual
- Meets with: direct support ULP staff, Social Worker, Marketing, Director of Operations, CEO regularly.
- Maintains an up-to-date knowledge of current trends of Home Care.
- Is responsible for investigating and reporting incidents of suspected client neglect and/or abuse and reporting these findings to Operations and/or appropriate county officials.
- Works to maintain privacy and confidentiality of client, client's records and client's living environment.
- Is willing to carry a cell phone and be on call for emergencies and notification of changes from ULP and management
- Communicates and documents client data to staff as needed.
- Completes initial assessment of potential clients to assure that their needs will be met.
- Completes assessment within two weeks of admission.
- Assures clients maintain residency requirements, reassess and recommend alternative placement if necessary.
- Assures clients have up to date care plans. Reviews and revises care plans as needed.
- Monitors and maintains client charts and nursing notes.
- Communicates to physicians and other personnel (county caseworkers) any changes in client's needs or conditions immediately.
- Monitors and follows thru with ordering medications and all medication renewals as well as any necessary follow-up with physicians regarding medications and/or medical conditions.
- Assures all physicians orders are signed and implemented with-in 24 hours.
- If needed, fills weekly pillboxes for medication administration by unlicensed staff and checks new medications from the pharmacy. Works closely with pharmacy to insure that all new medication cassettes received are accurate and filled correctly prior to placing on floor for use by ULP.
- Assures new medication sheets are correct/up to date in place by the first of each month.
- Reviews each client's medication sheet at least monthly or at time of any medication changes and assures accuracy and appropriate signatures.
- Communicates in staff communication book any information that staff needs to be aware of.
- Documents disposal of medication per procedure.
- Assures compliance with regulations and will participate in state licensure surveys.
- Assists in the development of policies and procedures and implementation of them.

Pay: $30.00 - $33.00 per hour
