As the Social Work Designee you will assist in planning, organizing, implementing, evaluating, and directing the overall operation of the Social Services Department in accordance with current federal, state, and local standards, guidelines and regulations, as well as our established policies and procedures.

Education and Experience
- Bachelors Degree from an approved school of Social Work preferred
- Active Social Work Assistant License acceptable
- 2 year supervised social work experience in a health care setting working directly with individuals preferred
- Long term care experience preferred
- Discharge planning experience preferred
