Parkview Care Center of Fairfield, Iowa is seeking a Social Services Director. We are a 70 Bed Skilled Nursing Facility. The Social Worker plays a key role in the social assessment and care plan development for each resident upon admission. Identify medically related social and emotional problems and potential for discharge. Assist with transportation arrangements to medical appointments and assist with the Medicaid application process. The Social Worker is responsible for complying with and interpreting to the residents, as necessary, the state and federal regulations for nursing homes concerning rights and facility guidelines.

Pay: $20.00 - $25.00 per hour
