TITLE: Assisted Living Director/CNS

REPORTS TO: VP of AL Operations

Job Summary: The role of the Assisted Living Director/CNS is to ensure the health and safety of all through routine onsite visits with the residents and staff. The Assisted Living Director will ensure that that the facility is in compliance with current regulations and standards. Must be able to relate professionally with residents, staff and family members, display a positive work attitude and be able to work as a team with other staff members. Assist with the marketing of the program.

Qualifications: Must be a Licensed Assisted Living Director within the State of Minnesota or be willing to obtain it. Must be registered and licensed with the State of Minnesota as an RN and must maintain licensure. Must have o willing to get a Food Manager License. The Assisted Living Director is also required to have an ability to read, write and speak English, and complete and pass a state required background study form which checks for certain criminal convictions or serious/recurring maltreatment.

General Responsibilities:

  • Completes focused assessments and implements service agreements with all new residents. Reviews and revises service plan as needed and a minimum of 1 time per year.
  • Assures residents have up to date care and service plans.
  • Assist with monthly resident billing.
  • Monitors and maintains resident charts and clinical progress notes.
  • Communicates to physicians and other personnel (county caseworkers,), family and/or designated representative, changes in resident's needs or conditions.
  • Facilitates coordination of resident services with other service providers within the current service limitations as described in the UDALSA.
  • Is responsible for ordering medications and all medication renewals as, well as any necessary follow-up with physicians regarding medications and/or medical conditions.
  • Ensures all aspects of medication management services are completed for all applicable residents.
  • Assures all physicians orders are signed and implemented with-in 24 hours.
  • Fills pillboxes for medication administration by care attendants.
  • Assures that medication administration procedures are followed properly and discusses any needed corrections, errors, etc., with staff.
  • Reviews each resident's medication sheet monthly and assures accuracy and appropriate signatures.
  • Communicates any information that staff need to be aware of.
  • Preforms staff supervisory visits within 30 days of employment and at least annually thereafter as delegated.
  • Works as a team player with all members of the resident’s care team.
  • Provides health and medication training to all new employees as well as continued education for all staff and documents such training to assure compliance with state, federal and local regulations.
  • Supervises medication practices and medical equipment being used in the building.
  • Assures that staff clearly understands and follow all resident care procedures.
  • Monitors staff for accuracy of monthly resident blood pressures, weights and pulse as requested.
  • Shares resident and family concerns with pertinent staff if appropriate.
  • Interviews, assigns, evaluates, orientates, and supervises all employees in the facility.
  • Employee scheduling and approval of time sheets as directed.
  • Assists in the development of policies and procedures and implementation of them throughout the facility.
  • Works with staff to assure emergency procedures are in place.
  • Completes assessments per regulations.
  • Is flexible and willing to expand and adapt to changes in the industry.
  • Promotes a positive, team approach and a healthy work environment
  • Is available for on-call medical questions and/or emergencies via phone. Occasional need to assist in house when on call.
  • Assures compliance with regulations and is available for federal and state licensure surveys.
  • Manages facility costs respecting the budget prepared for the facility.
  • Consults with the dietician (dietary consultant) regarding operating problems, patient food service, and therapeutic diets.
  • Instruct employees in use, care, and maintenance of equipment throughout the facility.
  • Supervises sanitation and housekeeping procedures.
  • Develop knowledge of actions to be implemented in emergency situations.
  • Participates in disaster planning drills (fire, tornado, severe weather, etc.) when on duty.
  • Practice fire safety and accident prevention procedures.
  • Must understand and adhere to all Vulnerable Adult Abuse Prevention Policies.
  • Adheres to HIPAA privacy rules and regulations.
  • Must obtain 8 hours Dementia Training within 160 hours of start of employment
  • Obtain required Dementia Training annually thereafter
  • Other duties assigned by the VP of AL Operations.
