The Licensed Social Worker is responsible for the implementation of standards of care for medical social work services and for adherence to all conditions in the Service or Employment Agreement. Assesses the psychosocial status of residents and families/caregivers related to the resident's condition and environment and communicates findings to the Registered Nurse and other members of the interdisciplinary team. Carries out social evaluations, including family dynamics, caregiver abilities, communication patterns, high risks for suicide, neglect or abuse and plans intervention based on evaluation findings. Counsels resident and family/caregivers as needed in relationship to stress, and other identified coping difficulties. Assesses for, and educates interdisciplinary team, on any special needs related to the culture of the resident and family. Includes communication, role of family, space, and any special traditions. Maintains clinical records on all patients referred to social work. Educates residents and families on, and assists in, preparation of advanced directives. Provides information and referral services for residents and families/caregivers regarding practical and environmental needs. Provides information to residents and families/caregivers and community agencies. Serves as liaison between residents and families/caregivers and community agencies. Maintains collaborative relationships with organization personnel to support resident care. Participates in the development of the individualized plan of care, involving the resident and family, and attends regularly scheduled interdisciplinary team meetings, assisting the team in recognizing the effects of the psychosocial stresses. Assists physician and other team members in understanding significant social and emotional factors related to health problems and death/dying issues. Actively participates in quality assessment performance improvement teams and activities. Other duties as delegated by the Administrator.

Responsibilities:
Works in close collaboration with physicians and other healthcare professionals in resident evaluation and treatment to further understand significant social & emotional factors underlying patients health problems.
Help residents and family through individual or group conferences to understand, accept, and follow medical recommendations.
Provides service plans to restore resident to optimum social and health adjustment within resident's capacity.
Prepares patient service plans and reports.
Utilizes community resources to assist resident to resume life in community or to learn to live within limits of disability.
Plans for improving health services by interpreting social factors pertinent to development of program.
Prepares all admission paperwork for incoming skilled and long-term care residents.
Coordinates all discharge paperwork, home services and medical equipment needed when going home.

Pay: $22.00 - $24.00 per hour
