What are the responsibilities and job description for the LPN Care Coordinator position at Parkview Primary Care Physicians PLLC?
Growing, well established WNY Primary Care Practice seeks NYS Licensed Nurse. Candidate must maintain a high level of patient interaction/service skills. Be self-motivated, compassionate and clinically strong. Ability to triage patient and report to clinician. Provide medical direction to patients per clinician recommendation(s). Other responsibilities would include referral/test tracking, tele-health visits regarding new medication follow-up, blood pressure and glucose readings, medical reconciliation, and any other duties assigned by manager. The Care Coordinator works in collaboration and continuous partnership with chronically ill or "high-risk" patients, their family/care giver(s) and their physician.
A typical day for the Care Coordinator will entail phone calls, portal messages and office visit with these "high-risk" patients.
Duties & Responsibilities include:
- Assess patient's unmet health and social needs
- Develop a care plan with the patient, family/caregiver(s) and providers
- Monitor adherence to care plans, evaluate effectiveness, monitor progress
- Reduce emergency room utilization and hospitalization re admissions
Education / Experience:
- NYS Licensed Practical Nurse
- Evidence of essential leadership, communication, education and counseling skills
- Highly organized with ability to keep accurate notes and records
- Medent EHR
- Proactively continues to educate self on providing quality care and improving professional skills
Job Type: Full-time
Pay: $26.00 - $28.00 per hour
