Bluffton / Warren, IN
Fort Wayne, IN - multiple locations throughout the city

Georgetown

Southwest - Illinois Rd.

New Haven

Rudisill

LaGrange, IN

Ligonier, IN

Noble / Kendallville, IN

Goshen, IN

Wabash, IN

Warsaw, IN

Details :

Hours of practice are usually Monday-Friday, 8 a.m. - 5 p.m.; some extended hours may be required (varies by location)

Light physician call - the call center allows for manageable call with excellent work / life balance!

Outpatient only
