MDS Coordinator for Parkwood Skilled Nursing, Maryland Heights
We are looking to hire a n experienced MDS nurse to work at our Skilled Nursing Facility. You would be working with the interdisciplinary team to create resident care plans, updating assessments, and monitoring and updating reporting of Case Mix and ADL's for residents.
MDS Coordinator Essential Duties
- Assessing and monitoring patients' health in accordance with medical laws, regulations, and guidelines.
- Collecting patient data and organizing the information into reports for further research.
- Oversees and evaluates the gathering of information for assessments and care plans, schedules and facilitates care plan conferences
- Monitors and guides the completion of PPS, OBRA assessments, Triggers and RAPs
- Transmit completed MDS to appropriate parties on a weekly basis; maintains 15 months of completed MDS on all residents for surveyors
- Performs the function of a nurse when needed
- Develop and maintain an ongoing quality assurance program for the resident assessment and care plans

- Help fill shifts on the floor if needed
- Take rotation of on-call phone responsibility
MDS Coordinator Qualifications and Experience
- RN or LPN for the state of Missouri
- Prior experience with MDS coding and processes
