Dawn to Dusk Wellbeing Center, is a new and well established DDD provider with a mission to serve adults with Intellectual and Developmental Disabilities at their home, out in the community and at our Day Program. We are currently looking for a Program Coordinator with a minimum of bachelor's degree. The Program Coordinator is responsible for supervising Direct Support Professionals in facilitating and carrying out the Day Program Activity Schedule for Dawn to Dusk Program participants. The Program Coordinator will work with the Program staff to ensure that the clients receive the individualized prescribed programming in a setting that supports the personal needs and goals of each participant’s Individualized Service Plan. Our Day Program hours are 8:30 AM to 2:30 PM. You will work approximately 8am to 4.30pm.
Under the firstion of the Director of Program Management, the Program Coordinator is responsible for the following:
- Day-to-day supervision and training of Direct Support Professionals in use of strategies for development of each individual’s behavioral, communication, and skill goal. This requires a thorough knowledge of each individual’s Individualized Service Plan (ISP), and the ability to teach and communicate effectively with Direct Support Professionals.
- Facilitation of the Program Calendar of activities and outings for all individuals attending the program. Coordinate with Direct Support Professionals to ensure activities are appropriate and engaging, group sizes are manageable, and that supports are readily available for the individuals and the Direct Support Professionals to effectively participate in the calendar of activities.
- Publishing and distributing a Program Calendar on a monthly basis that is designed for teaching community integration skills and providing social and recreational opportunities. Requires daily communication with all Direct Support Professionals assigned to the Program.
- Ensuring that the individuals are transported to and participate in these activities with full involvement from the Direct Support Professionals who are responsible for each individual’s care and support. The Day Program Coordinator should be actively involved with the scheduled activities and events.
- Reviewing the individual’s daily Therap notes submitted by the Direct Support Professionals to ensure completion and thorough reporting of the day. This may require providing feedback to the Direct Support Professionals on the quality and content of notes, as well as ensuring that the notes are completed within the 24-hour window.
- Coordinate and assist with coverage for Direct Support Professional breaks and absences.
- Participate in all ISP renewal meetings and complete all required paperwork including ISP worksheet, AENF if applicable, Individualized Goals and Strategies form, Annual update form, and processing of new ISP program and Service Authorization through Therap.
- Participate in all IDT meetings and individual update meetings, compile meeting minutes if needed, and complete all required paperwork such as ISP revision worksheet as needed.
- Participate in bi-monthly administrative team meetings and provide updates on the GAP Program, behavioral concerns and challenges on a program and individual basis, and efficacy of program calendar and activities.
- Collaborate with the BCBA and the Behavior Support Team to ensure effective implementation of behavior support strategies and guidelines, compile behavior data as prescribed and provide monthly updates on individual progress, and ensure Direct Support Professionals are following through on implementation and collection of the same.
Supervision:
The Director of Program Management is the direct supervisor of the Program Coordinator. The Program Coordinator will also be expected to work cooperatively with the Day Habilitation Activities Director, Behavior Support Team, and Direct Support Professional Supervisor of Dawn to Dusk.

Pay: $43,000.00 - $48,000.00 per year
