GENERAL SUMMARY:
Shift: Part Time 20 Hour Day Shift
Location: Bingham Farms, MI
The Medication Therapy Management (MTM) Analyst assists the departmental clinical pharmacists and other staff in the operations of the MTM Program.
PRINCIPLE DUTIES AND RESPONSIBILITIES:
- Telephonic outreach to patients eligible for the HFHS/HAP MTM Program to obtain patient enrollment and collect relevant medical and medication data.
- Telephonic outreach to improve patients’ knowledge of targeted drug(s)/disease(s) for drug adherence metrics associated with quality bonus payment structure.
- Entering pre-identified data in MTM Program documentation tools (e.g. Access, Excel, McKesson documentation tool).
- Liaison between patient eligible or enrolled into MTM Program and assigned clinical pharmacist.
- Educating MTM Program members about their Health Alliance Plan pharmacy benefit and evaluating HAP prescription drug claims processor data.
- Preparation and distribution of reports, mailings, letters, and other communications related to MTM Program.

.EDUCATION/EXPERIENCE REQUIRED:
- High School Diploma or G.E.D. equivalent.
- Exemplary oral and written communication skills. Previous call center experience strongly desired.
- Ability to use office productivity and departmental computer systems effectively. Ability to create and effectively utilize spreadsheets and databases is desirable.
- Previous retail pharmacy, hospital or managed care pharmacy experience is strongly desired.
- Previous work experience with senior citizens (individuals over 65 years of age) is strongly desired.
- Previous patient healthcare navigator experience is strongly desired.
