Primary Purpose of Position:  

The Re-Entry Peer Support Specialist provides support for individuals with serious mental illness (SMI) and specific charges, or who are prescribed clozapine, and are on the Department of Adult Correction (DAC) prioritization list and are being released from a DAC facility. This role involves offering peer support, facilitating access to community resources, and assisting the transition from prison to community-based care.


Role and Responsibilities:   

Peer Support:

    Offer emotional support and guidance based on personal experience with mental health and/or substance use recovery.

    Build trusting relationships with individuals to encourage engagement in treatment and recovery services.

    Share personal recovery stories to inspire hope and demonstrate the possibility of recovery.

In-Reach Services:

    Conduct regular meetings with individuals in prison or in the community, assess their needs, and develop personalized care plans.

    Provide education on mental health, substance use, and available community resources.

    Assist with pre-release planning to ensure a smooth transition back into the community.

Resource Coordination:

    Connect individuals with appropriate community resources, including housing, employment, healthcare, and social services.

    Coordinate with community agencies to secure necessary services and support for individuals upon release.

    Assist with scheduling and attending appointments as needed.

Advocacy:

    Advocate for the needs and rights of individuals within the criminal justice system.

    Work with prison staff, legal representatives, and community partners to address barriers to care and support.

    Promote a recovery-oriented approach within the prison and community settings.

Documentation and Reporting:

    Maintain accurate and timely records of interactions, care plans, and progress notes.

    Prepare and submit required reports to supervisors and other stakeholders.

    Ensure compliance with all confidentiality and documentation standards.

Crisis Intervention:

    Provide immediate support and intervention during crises to ensure safety and stability.

    Utilize de-escalation techniques and collaborate with crisis intervention teams as necessary.

Training and Education:

    Participate in ongoing training to enhance knowledge and skills related to mental health, substance use, and peer support.

    Provide training to prison staff and community partners on the role and value of peer support.

Collaboration:

    Work closely with the Re-Entry Care Coordinator and other stakeholders to ensure integrated and comprehensive support.

    Participate in team meetings and case conferences to discuss individuals' progress and coordinate care


Knowledge, Skills and Abilities: 

    Lived experience with mental health and/or substance use recovery, and willingness to share personal recovery story.

    Strong interpersonal and communication skills.

    Knowledge of mental health and substance use disorders, treatment options, and community resources.

    Ability to build rapport and maintain professional boundaries.

    Strong organizational skills and attention to detail.

    Ability to work independently and as part of a team.

    Proficiency in basic computer skills, including Microsoft Office


Education/Experience Required: High school diploma or equivalent required.  NC Peer Support Specialist certification required. One-two years of experience working with people with severe mental illness (SMI) and their families/guardians.

Ability to travel to prison facilities and community appointments.

Must live in North Carolina or within 40 miles of the border


Education/Experience Preferred:  

Some college coursework in human services or related field preferred. Experience working with individuals involved in the criminal justice system is a plus.


Licensure/Certification Requirements:  

NC Peer Support Specialist Certification
