Position Title: Healthcare Services Liaison
Supervised By: Executive Director
Location: High Point, NC
Position Description: The Healthcare Services Liaison position is part of a pilot program for the High Point area. This program and the associated position are intended to assist those experiencing homelessness with navigating the medical system while also assisting medical personnel with better understanding the homelessness system and the ways in which to assist patients experiencing homelessness.
Duties & Responsibilities:
- Builds relationships with medical providers in the High Point area for purposes of raising awareness about available resources and existing systems that are in place to assist people experiencing homelessness.
- Assists in connecting medical providers to the appropriate programs and providers to ensure patients are assisted in an efficient and effective manner.
- Establishes and maintains a High Point Homelessness and Health Care (HPHH) committee, which will be comprised of various stakeholders.
- Complete ongoing analyses of the existing systems, noting any needs and gaps.
- Maintains and reports data and information.
- Conducts trainings for various stakeholders to improve communication between systems.
- Interacts appropriately and professionally with internal and external customers including individuals receiving services, colleagues, community service providers, and other department professionals.
- Provides resources and/or referrals as appropriate.
- Connects individuals with services based on identified needs.
- Maintains current and accurate logs of client interactions.
- Maintains communication with supervisor relative to program functioning, unusual events, problems, as well as successful achievements.
- Other duties as assigned.
Benchmarks/Success Measures:
- Establishment of a High Point Healthcare and Homelessness Committee.
- Creation of resource-based toolkits for a variety of audiences.
- Development of a system intended to provide resources to callers as appropriate.
- Maintenance of systems noting calls received and information requested or provided.
- Completion of multiple needs and gaps analyses/community assessments regarding healthcare and homelessness.
Education & Experience:
- Bachelor’s degree preferred.
- Two years of customer service experience in a fast-paced environment is preferred.
- Knowledge of the medical system preferred.
- Experience using Microsoft Office Suite, Google Workspace, and Dropbox.
- Excellent rapport building and networking skills.
- Ability to work with diverse populations.
- Excellent communication and presentation skills (verbally and in writing).
Job Type: Full-time
Pay: $30,000.00 - $35,000.00 per year
