What are the responsibilities and job description for the Program Coordinator position at Partners for Quality Health?
Overview
The Program Coordinator’s primary responsibility is to assist the department head to support our client practices to succeed in various programs.
This will include actively participating and supporting clinical teams by assisting with practice development and transformation, workflow improvement and implementation, patient outreach, physician and staff training, data entry, and other support functions to ensure the succeed of the physician practice.
RESPONSIBILITIES include, but not limited to, the following:
- Provide information, resources, and training to help client practices succeed in various programs
- Support practices with guidance and assist practices in workflow improvement and implementation
- Identify problems or issues and provide solutions
- Use PDSA model and evidence-based guidelines to help practices with quality improvement, practice development and transformation
- Develop mechanism to support changes in the practices to ensure sustainability
- Document quality improvement projects, successes, challenges, barriers, resolutions, lessons learned, etc.
- Facilitate meetings and webinars to provide information and training for practices
- Engage practices in collaboration, networking, and learning.
- Assist with development of programs and activities to assist with patient care and program success
- Assist with clinical integration with primary care and specialty care
- Coordinate ecosystem and community programs to ensure practice success
- Maintains strict confidentiality
- Data entry of highly sensitive clinical information for client practices
QUALIFICATIONS:
- Bachelor’s degree and 3 years of related work experience required
- Experience in clinical workflow and business functions
- Strong management skills
- Strong analytical skills
- Strong presentation and training skills, with the ability to effectively communicate complex information and help physicians and staff understand
- Effective verbal and written communication skills
- Proficiency with Microsoft Office products; e.g. Excel, Word, PowerPoint, etc.
- Have adequate transportation to and from client practices and other locations
- Available to work during our business hours, evenings and weekends when required
Job Type: Full-time
Pay: From $45,000.00 per year
