LOCATION: Robinson Township - Travel will be required based on the needs of the people receiving life sharing supports. We currently support people in Allegheny, Washington, Beaver, Butler, Westmoreland, Erie, and McKean Counties.

Responsibilities
The Life Sharing Nurse serves as a member of the Life Sharing team reporting to the Director of Life Sharing. The Life Sharing Nurse is responsible for assisting with the provision of health and wellness care to adults and children with intellectual disabilities and autism who receive support in both typical and medically complex (MCC) life sharing programs. The Life Sharing Nurse assists the Life Sharing Coordinator with completing the Health Risk Screening Tool (HRST) for all people receiving life sharing support. Responsibilities will include working collaboratively with the Life Sharing Medical Coordinator to assist in coordinating services to support the medical, health, and wellness needs for people supported. The Life Sharing Nurse will bring a positive and person-centered care approach and work cooperatively with administrative and program staff. This position will require travel to the homes of people supported for home visits and will have the flexibility to telework from home on a Citizen Care device as well as in the administrative office when not in the field as scheduled with the Life Sharing Medical Coordinator.

Health & Wellness
- Demonstrate a working knowledge and understanding of personal outcomes for people supported, especially as they apply to their health, wellness, and medical needs.
- Advocate for people supported to achieve their personal health and wellness goals through education and consultation.
- Assist the Medical Coordinator with implementation/integration of the HRST; ensure routine up-dates and proper documentation within the screening tool as required.
- Work with team members to include pertinent service considerations generated by the HRST in the person’s ISP.
- Develop and provide training related to medical topics or identified medical needs of people supported across all programs.
- Complete and maintain certification as an HRST rater and clinical reviewer.
- Serve as the clinical reviewer for completed HRSTs within the life sharing programs.
- Demonstrate familiarity with Office of Developmental Programs’ (ODP) Medication Administration Process for Life Sharing Partners; review medication errors when needed and provide training to ensure proper medication administration protocols are followed.
- Conduct initial meetings with the people referred and their families/caregivers, assist with collection of required information as necessary.
- Utilize the electronic health record to assure health care summaries including monthly progress notes for children in the MCC life sharing program and people supported in the life sharing program, Medication Administration Records (MARs), medical/nursing notes, social medical histories, and other medical documentation within medical records are accurate and current.
- Display a strong commitment to the accomplishment of CCI’s Mission and Vision.
- Support and actively model values associated with The Council on Quality & Leadership (CQL) and Everyday Lives.
- Work collaboratively with the Medical Coordinator and the CCI nursing team to develop strategies to proactively close identified care gaps-Healthcare Effectiveness Data and Information Set (HEDIS).
- Collaborate with the Medical Coordinator to identify, track, and analyze health and wellness trends/outcomes utilizing data from HRST and other associated health and wellness programs in place at CCI.
- Participate in Quality Management activities, including the development of Business Information dashboards, as assigned.
- Responsible for the completion of other related duties as assigned.

Qualifications
- Must be a Licensed Practical Nurse (LPN) or Registered Nurse (RN) with a current Pennsylvania license
- Four (4) years experience in nursing required
- Experience in Intellectual Disabilities, Autism, and Mental Health Services preferred
- Experience in dealing with emergency and/or acute medical situations preferred
- Valid PA driver’s license
- Use of personal vehicle required
- Act 33 / 34 and FBI Clearances (working with children)
- Excellent references, both personal and work
