The primary focus of this role is to empower individuals navigating their behavioral health journey. Social Rehabilitation service is designed to improve overall health outcomes by targeting key areas of Holistic Health focusing on the following areas:
- Supportive person-centered activities that promote safe and sustainable independent living
- Opportunities to strengthen community engagement and social support for those served
- Educational activities and health literacy promotion
- Ensure access to healthy food and nutrition
- Improve economic stability through exploration of employment related activities
This multifaceted role involves working in tandem with Behavioral Health Therapists and Social Activity Coordinators to help plan and lead therapeutic programming and facilitates the implementation and co-facilitation of group-based activities both within the facility and in the community. By assisting with providing support and tailored interventions, you will play a vital part in helping individuals overcome challenges and develop essential life skills.
- Coordinating and managing therapeutic activities, including scheduling, securing needed supplies and transportation, budgeting, and resource allocation
- Co-facilitate dynamic and engaging activities or discussions that promote personal growth, self-awareness, and skill development, honoring the diverse perspectives and experiences within the group which meets individualized objectives
- Provide people supported activity opportunities to foster skills, identify integrative community activities and coordinate resources for participation
- Comply with Crisis Plans, Support Plans and Documented Accommodations for people supported guided by the program’s Behavioral Health Therapists and Supervisor to ensure best practice and consistency
- Encouraging and assisting people served with participation in activities, providing support and encouragement as needed
- Consults with Social Rehabilitation Supervisor or Director and other Milestone representatives frequently regarding client behaviors and potential needs
- Maintaining a safe and welcoming environment for all people served
- Complete documentation in a timely manner as needed
- Complies with all Milestone Program Policies/Procedures and Partners for Quality and Milestone Specific Personnel Policies and Procedures
- Completes all other duties as assigned
