Primary Purpose of Position: The I/DD Assessment Specialist is responsible for providing individual-specific assessment, including the Supports Intensity Scale (SIS) for individuals with intellectual and/or developmental disabilities. This employee is responsible for providing supports-oriented assessment/evaluation to an assigned group of individuals. This role requires a high degree of professionalism and fidelity to assessment protocols. Travel is an essential function of this position.
Role and Responsibilities:
The I/DD Assessment Specialist is responsible for (though not limited to):
Assessment
- Serves as Supports Intensity Scale (SIS) Interviewer, ensuring accurate and timely assessment of support needs for adults and/or children.
- Ensures that respondents for SIS interviews meet the required criteria and include, to the maximum extent possible, those who spend the most time with the individual supported
- Ensures that individual/legally responsible person, assigned Care Manager and service providers receive a copy of the results of assessment
- Completes other assessments as assigned
- Consistently follows established internal protocols, as well as assessment-specific protocols
- Demonstrates ability to provide supportive/effective individual-specific consultation to Care Managers and/or planning teams on how to use assessment results to inform support planning
- Exemplifies positive solution focused attitude
- Promotes problem solving and a positive, goal-oriented partnership with other employees, individuals supported, legally responsible persons, service providers, etc.

Documentation/Fiscal Accountability
- Demonstrates ability to achieve established productivity targets, currently completion of minimum of 17 SIS assessments per month on average
- Demonstrates ability to gather, analyze, and report information in an effective manner.
- Proactively monitors own documentation to ensure that issues are resolved as quickly as possible.
- Uses available agency resources (e.g. laptop, mobile phone) in efficient manner and in compliance with all requirements (e.g. work/business use only, secure storage, etc.)
- Completes all required paperwork to document assessment/planning processes and consultation/education provided (e.g. assessment documents).
Knowledge, Skills and Abilities:
- Comprehensive knowledge of the assessment and treatment of I/DD needs, with or without co-occurring physician health or physical health needs
- Knowledge of the Supports Intensity Scale and a strong commitment to quality, accurate assessment
- Ability to consistently demonstrate inter-rater reliability in administration of both the Child and Adult SIS
- Ability to facilitate assessment interviews/processes in an efficient manner, following established protocols
- Ability to understand and consistently follow established protocols
- Ability to gather, objectively analyze and report information in an effective manner
- Highly effective communication ability, both oral and written
- Propensity to make prompt independent decisions based upon relevant facts and established processes
- Problem solving, negotiation and conflict resolution skills

Education/Experience Required:
Bachelor’s Degree in a field related to health, psychology, sociology, social work, nursing, or other relevant human services area or licensure as a registered nurse (RN) and two (2) years post-degree of experience working directly with individuals with I/DD. Must additionally have at a minimum two (2) years of prior LTSS and/or HCBS coordination, care delivery monitoring and care management experience. This experience may be concurrent with the two years of experience working directly with individuals with I/DD described above.
Must have ability to travel regularly as needed to perform job duties.
Education/Experience Preferred:
Experience with NC Innovations Waiver, quality improvement processes, and specifically with the Supports Intensity Scale preferred. Current certification as an Interviewer for the Adult Supports Intensity Scale and/or Child Supports Intensity Scale preferred.
Education working with Individuals with both I/DD and physical health or behavioral health needs preferred.
Licensure/Certification Requirements:
Must attain/maintain status as certified Supports Intensity Scale Interviewer
Supports Intensity Scale for Adult (SIS-A) Interviewer:
Initial SIS-A certification should be successfully obtained within 90 days of completing the training period.
Supports Intensity Scale for Children (SIS-C) Interviewer: (as assigned)
Initial SIS-A certification should be successfully obtained within 90 days of completing the training period.
If a Registered Nurse (RN), must be licensed in North Carolina.
