Compensation : $40.00-$57.64 (DOE).
Sign-on Bonus : $2,000, paid after 90 days of employment.
Location : Central Oregon.
Hours : Full Time . Monday-Friday 8-5; 4-10s available.
Travel : Local / regional travel by personal vehicle during the working day, centralized from the Bend office and extending throughout Central Oregon. Mileage reimbursement is provided.
What You Get to Do :
Our Support Registered Nurses plan, coordinate, and evaluate home health and hospice services. You will focus on community health education and use available resources to provide high-quality, cost-efficient care to individuals and families at home. Ideal candidates have experience in home health, hospice care, and community health education.
- Complete a comprehensive and accurate assessment of the patient and family to determine home health needs.
- Coordinate patient care with the patient’s case manager. Provide verbal or email updates as necessary.
- Provide a complete physical assessment and history of previous illness (es) and use the health assessment data to determine the appropriate diagnosis and need for other disciplines and resources.
- Provide nursing procedures and treatments and end-of-life comfort procedures per care plan. Administer medications and treatments as prescribed by the physician.
- Provide health care instructions and education to the patient and or family as appropriate per the assessment plan and patients’ caregivers’ ability to learn. Adapt teaching styles to meet patient’s needs and family’s needs.
- Communicates with the physician regarding the patient’s needs, reports, and changes in the patient’s condition; obtains / receives physician’s orders as required.
- Communicate with all healthcare team members to promote coordinated, efficient care and document coordination of care with all involved with patient care.
