Job Summary:
We are seeking an RN for a temporary assignment in our Ronan office to help provide care to patients in Lake County. We can be flexible and offer opportunities for 8, 10 or 13 week schedules. This position is not benefits-eligible and will not include overtime.
The RN in Ronan provides skilled nursing, care for assigned patients in their place of residence; assists with coordination and integration of services to ensure continuity of care; facilitates the achievement of quality, clinical and cost outcomes; negotiates, may procure and coordinate services and resources needed by the patient or family; creates opportunities and systems to enhance outcomes; and uses the nursing process of assessment, planning and implementation of care, including patient/family instruction and evaluation through an inter-disciplinary team approach.
Essential Duties and Responsibilities:
- Provides skilled nursing and coordinates care to patients and family members in a residential setting through a team approach and based upon systematic evaluation as authorized by a physician.
- Observes, assesses, evaluates, and plans interventions in relation to patients’ physical and psychosocial statuses, including changes in condition and/or reactions to treatments or medications for the duration of contact with patients.
- Evaluates compliance with plan of care provided by Home Health Aides to assigned patients.
- Assures continuity of care through effective communication, information exchange and case conferences with other disciplines, community health, support and social agencies.
- Provides quality care and appropriate symptom management for positive clinical outcomes and promotes optimal health and quality of life through teaching, counseling, appropriate interventions and referrals.
- Maintains accurate, current and complete patient records in accordance with Agency standards and OASIS requirements as well as knowledge of current reimbursement rules and regulations.
- Participates in team scheduling coordination and “on-call” rotation schedule as necessary to meet patient care needs by assessing and evaluating patient needs via telephone and advising or making home visits as required.
- Participates in quality improvement activities and attends in-service training as assigned.
Minimum Qualifications:
Qualifications include a current RN license in the State of Montana (BSN preferred), a valid driver’s license, appropriate auto insurance, and reliable transportation. 2 years of recent acute care or home care experience including such clinical skills as venipuncture, catheterization, wound care and/or parenteral therapy preferred.
Schedule:
Ronan RN Case Managers are required to provide a higher level of support than Missoula RNCM staff as follows:
- Weekend rotation more often than RN in the Missoula office. This responsibility is expected to require rotation every 2-3 weeks as staffing allows.
- On-call responsibilities need to be covered a week at a time. This responsibility is expected to require rotation every 3-4 weeks as staffing allows.
- Provide care to patients in both Home Health and Hospice as community need dictates.
- Provide patient admissions as needed to maintain home health and hospice services in the community.
Salary : $60
