Job Summary:
The Home Health Referral Coordinator plays a key role in ensuring the efficient management and processing of patient referrals to home health services. This position requires a detail-oriented individual with excellent communication skills to coordinate with physicians, patients, clinical managers, Intake, centralized schedulers, and the home health teams. The coordinator will assess referrals for completion and accuracy, medical necessity, needed disciplines, physician orders and will process them through to be scheduled.
Essential Duties and Responsibilities:
- Referral Intake & Processing:
Receive and process patient referrals from physicians, hospitals, and other healthcare providers. Ascertain completeness and accuracy of patient information, including necessary chart attachments, medical necessity, disciplines ordered, order accuracy and document in the EMR. - Scheduling:
Coordinate with schedulers for initial assessments to meet regulatory compliance. - Coordination with Clinical Teams:
Communicate regularly with doctors, nurses, therapists, and other healthcare professionals to ensure continuity of care and appropriate service delivery for patients. - Documentation & Data Entry:
Maintain accurate and up-to-date records in the EMR. Ensure compliance with healthcare regulations, including HIPAA, when handling patient information. Completes workflow in EMR related to referral coordination and order review, timely and accurately throughout the day. - Internal and External Customer Service:
Act as the point of contact by managing incoming calls from patients, families, physicians, clinicians, and other health provider agencies. - Obtain, Track, and Enter Orders:
Ensure all patient orders (e.g., physician orders for home health services, equipment, and medications) are obtained, tracked, and entered into the Electronic Medical Records (EMR) system accurately and promptly. Confirm the receipt of orders from physicians, hospitals, and other healthcare providers. Verify completeness and clarity of the orders before entering. Monitor the status of orders to ensure timely processing and delivery of services. Such as tracking wound care orders from the clinic s/p patient visits. - On Call:
May participate in after-hours clinical on call to assist nurses with questions regarding patient after-hour needs. - QAPI:
May participate in Quality Assurance Performance Improvement plans and action items. - Safety and Environment:
Promotes safe work practices and environments to protect the safety and well-being of both employees and patients. - Supplies Management:
Provides support with supply management in ordering and review/approve tasks. - Additional Duties:
Perform other duties as assigned by supervisor.
Qualifications:
· Required: Current LPN or RN license in the State of Montana, a valid driver’s license, appropriate auto insurance and reliable transportation.
· Preferred: 1-2 years of previous LPN experience in acute care, home care or long-term care, including clinical skills of aseptic technique, wound care, catheterization, ostomy care, and bowel programs.
