AMERICORPS MEMBER POSITION TITLE: Community Health Coordinator (Bilingual preference English & Spanish)
HOST SITE: This AmeriCorps position will be shared between two organizations: United Way Fox Cities (1455 Midway Rd, Menasha, WI) and Partnership Community Health Center (119 N. McCarthy Rd, Appleton, WI).
ACTIVITY LOCATION: Hybrid – On-site and remote. Work will be performed primarily at United Way Fox Cities, Partnership Community Health Center, or out in the community. There is the option to perform some duties remotely, as agreed on between the member and supervisor. Please note, the member must live in Wisconsin and be willing to commute to the host site organizations.
OVERVIEW: The member will enhance community wellness efforts by supporting the Eat Well for Life Program and other Be Well Fox Valley community initiatives to support the Be Well Fox Valley vision and mission.
Additionally, the member will work closely with the Behavioral Health and Outreach and Enrollment teams at Partnership Community Health Center (PCHC) to serve as a liaison to community partners and improve access to care.

DESCRIPTION OF DUTIES AND RESPONSIBILITIES:
- Assist with identification and recruitment of community members who qualify for Eat Well for Life (EWFL)
- Support EWFL program operations by preparing class materials, connecting participants with diabetes-friendly foods, coordinating and attending health education sessions, and capturing participant data
- Communicate with program participants to help identify and assist with barriers and goals
- Work with food pantries to implement diabetic-friendly practices
- Create marketing and communication materials for Be Well Fox Valley
- Utilize PCHC’s electronic health record system to identify patients with care needs and connect them with appropriate services
- Serve as a liaison to community partners to educate and connect clients with PCHC services
- Support the PCHC Outreach and Enrollment team at community events
- Collaborate with the Behavioral Health team and community partners to reduce stigma and enhance utilization of behavioral health services at PCHC
- Attend and participate in BWFV AmeriCorps program trainings on a variety of topics, including program orientation all-member team training and networking opportunities, Public Health AmeriCorps CDC Train virtual training series, community service projects, etc. These trainings occur through a combination virtual and in-person.
- Complete AmeriCorps paperwork, such as monthly reports, feedback surveys, etc.
- Community outreach around health and wellness topics and objectives
- Deliver community-based programing around health and wellness topics and objectives
QUALIFICATIONS/SKILLS/EDUCATION/EXPERIENCE:
- The ideal candidate will hold an Associates or Bachelor's degree in a relevant health and human services, health promotion and wellness, public health fields, or related background and experience based on host site need. Upper-level undergraduates with related experience are encouraged to apply.
- Must be able to fluently speak Spanish and English
- Be 18 years or older at the beginning of their service
- Hold a high school diploma or equivalent
- U.S Citizen, U.S National, or Lawful permanent resident alien of the U.S
- Complete and pass an AmeriCorps-required background check
OTHER INFORMATION:
- Member Term of Service: The Member’s service term will start on 3/3/2025 and end on 8/31/2025.There’s flexibility with the position start date must start sometime between 3/3/25 and 3/24/25.
- Time Requirements: The member is expected to serve 675 hours, which averages 26-31 hours/week during the service term period.
- Benefits:
- Members will receive a gross living allowance while serving, which is paid on the 15 and last day of each month. The gross living allowance amount will be $761.03 per pay period (the amount listed is before tax deductions). Please note, there will not be an additional hourly wage or salary.
- Segal Education Award upon successful completion of hourly requirement described in Time Requirements: The Education Award amount for this position is $2,817.14. Please note, this education award is an electronic transfer to either your educational institution, federal loan provider, etc. Members will not receive the education award via direct payment (such as check, cash, etc.).
- Forbearance on qualified student loans while serving and interest accrual payments.
