What are the responsibilities and job description for the Intellectual Disabilities Supports Coordination Supervisor position at Partnership For Community Supports?
The Intellectual Disabilities Supports Coordination Supervisor will provide supervision and mentoring for a small team of Supports Coordinators. The ID SC Supervisor will assure that high quality Supports Coordination services are provided to the individuals that we support who have an Intellectual Disability or Autism.
Duties include: Develop positive person-centered working relationships with individuals, their families and provider agencies. Provide effective leadership for the program.
- Ensure Individual Support Plans are entered into the Home and Community Services Information System by Supports Coordinators within required time frames and in keeping with all Federal, State, and City compliance requirements.
- Assure coverage is provided during Supports Coordinator absences or vacancies.
- Connect with community resources and specialized services.

Requirements
- Bachelor’s degree required which includes a major coursework in Sociology, Social Work, Psychology, Gerontology, Criminal Justice, or other related Social Science and two years' experience as a Supports Coordinator OR have a combination of experience and education of at least six years of experience in developmental disabilities, special education, mental health, counseling, guidance, social work or health and rehabilitative services; and at least 12 college-level credit hours in Sociology, Social Work, Psychology, Gerontology or other related social science.
- Valid driver’s license and use of personal automobile required to complete home visits and program visits in Philadelphia County, Bucks County, Chester County, Delaware County, York County and Montgomery County. Please note that home and program visits are a requirement of your position.
- As we are subject to the City of Philadelphia’s COVID-19 Vaccine Mandate for Healthcare Workers, you will be required to be fully vaccinated against COVID-19.
- Computer skills and excellent communication and organizational skills needed.
- Job requires a sensitive, caring individual with excellent verbal and written communication skills who wants to make a difference in the lives of individuals with intellectual disabilities and their families.
- Bilingual a plus.
- Access to a personal, secure, password protected home WiFi connection preferred.
