What are the responsibilities and job description for the Medicare-Options Counselor position at Partnership for Public Health?
General Overview: The ADRC (Aging & Disability Resource Center), a program of the NH Department of Health and Human Services, partners with Partnership for Public Health (PPH). As New Hampshire’s designated Aging and Disability Resource Center and NHCarePath Full-Service Access Partner, the ADRC provides services for individuals of all ages, income levels, and abilities. They help connect people to long-term services and support, family caregiver information, options exploration, and Medicare and Medicaid resources. Responsibilities:
1. Outreach and Education: a. Participate in group outreach and education events, at a minimum, per scope of services with an emphasis on providing and distributing outreach information. b. Ensure adequate stocks of up-to-date print and video resources related to Medicare. c. Provide marketing coordinator with program updates and marketing events to be posted on social media.
2. Grant Compliance
a. Provide one-to-one counseling to clients on issues related to Medicare health insurance, including but not limited to, enrollment, understanding their benefits and billing statements.,
b. Participate as part of the ADRC team to provide information, referrals, assistance, and follow up to clients through home and community visits, telephone, walk-in, group sessions, or email as needed/appropriate.
c. Assures all electronic client records are maintained accurately by following documentation.
d. Assist the Center Manager in ensuring all ADRC required reports as well as coordinates record keeping and reporting for programs as required by CMS, ACL, BEAS, and ADRC. This currently includes Navigate, STARS and the ADRC Outreach and Education Report.
3. General:
a. Work collaboratively with the Center Manager to establish short and long-term goals to achieve the ADRC mission.
b. Oversee the Medicare program for the ADRC – Belknap County site. Medicare programs include State Health Insurance Assistance Program (SHIP) and Medicare Improvements for Patients and Providers (MIPPA.)
c. Performs other duties as assigned by the Center Manager, Human Services Administrator, Chief Operating Officer or Chief Executive Officer.
Education, Experience, and Skills Required:
1. Education & Experience:
a. Bachelor’s degree preferred or associate’s degree with equivalent experience.
b. Obtain SHIP Certification within 3 months of hire
c. Obtain certification as Information Referral Specialist (InformUSA) and Person-Centered Options Counseling within 6 months of hire.
2. Skills:
a. Experience in customer relations, call management, information and referral or related field that includes phone skills and preliminary assessment and triage ability of contacts.
b. Experience working with older adults, caregivers, veterans, and/or persons with disabilities, knowledgeable about local, state and national resources.
c. Moderate skills in: listening, customer service, interviewing, and understanding of services, advocacy, and documentation.
d. Strong written and verbal communication
e. Good interpersonal skills, openness and flexibility in working with diverse groups, and enthusiasm for working collaboratively with a team,
f. Good computer skills.
g. Ability to establish and maintain effective working relationships with groups such as: representatives of other social service agencies, institution officials, the public, and clients.
h. Ability to organize and present at educational and outreach events.
i. Ability to communicate effectively, one-on-one or in group situations, to explain complexities of Medicare, to summarize data, to prepare reports and to make recommendations based on findings which contribute to quality of SHIP and MIPPA programs, solving problems and meeting work objectives
3. Physical Requirements
a. Ability to work a flexible schedule, which may include night and weekends as well as being able to travel throughout the region and state as required. Additional hours may be required during Medicare Open Enrollment.
b. Minimal requirement to reach at or above shoulder level; occasionally reaching below shoulder level required.
c. Ability to tolerate prolonged sitting and/or standing and to lift up to approximately 25 pounds to waist high level whenever necessary.
