What are the responsibilities and job description for the County Child Welfare Liaison position at Partnership HealthPlan of California?
Overview
Provides a range of social work services as a county child welfare liaison in collaboration with Partnership staff to meet the psycho-social and care coordination needs of members involved in the child welfare system. Responsible for the assessment and care coordination of the psychosocial needs of these members, families and/or caregivers to help promote positive coping skills, reduce the risk of premature institutionalization, assist individuals in maintaining independencein the community and increase stabilization of social determinants.
Responsibilities
- Supports the Sr. Manager of Foster Care Programs with the administration anddeliverables of the County Child Welfare MOU.
- Attends County child welfare meetings, SOC 2083 ILT Meetings, State TechnicalAssistant Calls, MDT, and other internal and external meetings as appropriate.
- Provides formal presentations to both external and internal stakeholders on mattersrelevant to this population and regulatory requirements pertaining to it.
- Serves as a primary point of escalation for care when working with County andcommunity partners.
- Ensures covered services are closely coordinated with other services, including socialservices and specialty mental health care services.
- Travels and works with local child welfare agencies and ECM providers in Partnershipcounties to engage and create social and professional relationships.
- Responsible for educating and coordinating providers and teams on appropriate referrals.
- Supports the development of an ECM network specific to meet the needs of this childwelfare-involved population
- Serves as the primary lead point of contact on behalf of the Plan with external childwelfare serving stakeholders (county welfare departments, foster family agencies, CASA,foster families, etc.)
- Travels to meet child welfare-involved members, families, and/or caregivers in thecommunity to address case management and care coordination needs.
- Develops an individualized care plan addressing the psycho-social elements and nonmedical barriers to health in an effort to promote and ensure optimal care coordination ofthe member’s needs.
- Conducts interview assessments with individuals, families and/or caregivers to identify amember’s needs, goals and/or barriers.
- Sets priorities according to treatment goals and member’s preferences in a timely manner.
- Supports successful transitions and coordination of services across the care continuum(ex: pediatric to adult settings, acute/post-acute to community, adaptation to disability ortrauma, etc.) via effective and frequent communication and collaboration with providers,the child welfare-involved member, family and/or caregiver.
- Arranges, coordinates and monitors the use of community services/resources available tochild welfare-involved members, family and/or caregivers. (ex: housing options, financialassistance, special education options, vocational training/employment, legal aid services,etc.)
- Assists members in selecting appropriate resources and/or contacting resources on behalfof the child welfare-involved member. Provides support in obtaining necessary forms,filling out documents/applications when appropriate.
- Provides education and support to child welfare-involved members, families, caregiversand/or PHC staff regarding conservatorship, Public Guardianship, Power of Attorneyand/or Authorized Representative options for members with identified needs.
- Uses Motivational Interviewing strategies and clear communication/limit setting topromote member’s self-management, reduce risk of system(s) dependency, and improvehealth and well-being.
- Documents interventions and activities according to departmental policies andprocedures.
- Collaborates with colleagues to provide education and support to members withidentified psycho-social and/or resource needs.
- Participates as part of a multidisciplinary team in Partnership’s integrated rounds;presenting complex member cases when needed and offering suggestions or support toothers.
- Maintains working knowledge of public/private community resources and fosterscollaborative working relationships on behalf of Partnership.
- Actively participates in case conferences in the community, including hospitals, skillednursing facilities, community-based organizations, etc.
- Engages in essential skills training as directed or assigned.
- Provides training and/or on-boarding support to new staff as assigned and serves asliaison between direct line staff and supervisor
- Proactively identifies strategic partnerships and/or community-based activities to enhancepsychosocial supports to members and/or families.
- Fosters professional working relationships with providers and/or community-basedagencies on behalf of Partnership.
- Other duties as assigned.
Qualifications
Education and Experience
Bachelors or Master’s degree in Social Work, Minimum of three (3) tofour (4) years of social work in a foster family agency or child welfareserving agency preferred or equivalent work experience considered.
Special Skills, Licenses and Certifications
Valid CA driver’s license and proof of current Automobile insurancecompliant with Partnership policy are required to operate a vehicle andtravel for company business.
Performance Based Competencies
Experience with human growth and development, group and familydynamics, principles of individual and group behavior, provisions offederal, state, and local welfare programs and the laws, rules, andregulations governing their applications: community organizations andresources: socioeconomic conditions and trends: current problems andmethodology in public social service. Ability to: assess and evaluatedata and develop an effective course of action; make effective use ofresources and services in the community; develop and maintaineffective working relationships with members, departmental staff,public and private Social Service agencies, community basedorganizations, and the public; control personal bias and apply socialwork ethics; input data, reports and narratives into a computer system;keep detailed records and prepare reports and correspondence.Bilingual skills in Spanish preferred.
Work Environment And Physical Demands
Daily use of telephone, fax, computer, and photocopying machine isrequired. Standard cubicle workstation with a shared common area.Must be able to lift, move, or carry objects of varying size, weighing upto 10 lbs. Frequent travel and/or field-based work (at least 50% drivingtravel required).

HIRING RANGE:
$93,690.86 - $117,113.58
