Mon-Fri 9-5 pm, 8-4 pm, some nights and weekends required
FUNCTION
The Clinical Program Manager will supervise and provide intensive, comprehensive case management and referrals for all victims of human trafficking identified. The manager will oversee residential programming, including facilitating clinical groups for residential and community-based clients. The manager will work with regional service provider partners to increase collaboration, delivery of services, and coordination of all aftercare services for residents. The manager will conduct intakes and work with the residential team to determine eligibility for services. The manager will participate in meetings with the regional continuum of care housing providers and Connecticut Access to Network services to access vouchers, subsidized placements, and other resources for residential and community-based clients to access long-term housing solutions. The manager will strengthen community partnerships and develop rapport with local law enforcement to enhance collaboration across the state and increase PEHT’s impact in the community. The manager will be responsible for overseeing the OVC-funded housing grant to ensure the accurate execution of deliverables. The manager will be responsible for developing and participating in awareness and training events when applicable.
DUTIES
Program Management Responsibilities
- Conduct initial screening and intake of newly identified victims of HT and ensure all coordinated referrals are made with the Director of Housing.
- Participate in managing a 24 / 7 emergency after-hours response to victims of HT and coordinate services needed for all potential victims.
- Respond to partner calls and screening of potential victims of HT with local and federal law enforcement.
- Help develop housing resources and networks to support coordinating access to affordable housing for clients.
- Facilitate clinical groups for residential and community-based clients.
- Collaborate with OVC-funded partners and service providers in the Fairfield County region and the State of CT for ongoing coordination of services of OVC grant recipient clients.
- Participate in the ongoing planning and development process of the OVC Services for Victims of Human Trafficking grant, as well as data collection activities.
- Participate in the development of economic empowerment training for individual survivors of the PEHT residential program and provide linkages to the social enterprise employment opportunities.
- Host Life Skill programming and career development skill building.
- Continuously evaluate and update policies and procedures to ensure all program practices remain survivor and trauma-informed, including a commitment to the Sanctuary Model for trauma-informed care.
- Facilitate and support the Director of Housing with weekly clinical team meetings and case conferencing.
- Educate stakeholders and landlords around the existing housing protections that are available to victims. Provide trauma-informed education on how to best support this population regarding housing-related concerns.
- Assist victims in identifying, obtaining, and retaining suitable housing.
- Provide case management to individuals receiving housing assistance and prepare clients for independence by receiving financial literacy support. Aid in lease negotiation with landlords when needed.
