Key Responsibilities:
- Collaborate with the Registered Nurse (RN) to develop and implement individualized care plans.
- Assess patients/residents to identify their needs and preferences.
- Administer medications and perform treatments per physician orders.
- Document patient progress and communicate with the healthcare team.
Requirements:
- Graduate of an approved school for practical or vocational nursing.
- Currently licensed by the State Board of Nursing or pending licensed practical nurse licensure.
- Maintain current BLS/CPR certification if employed at a skilled nursing center or as required by the state of practice if an assisted living facility.
