What You’ll Do
✔ Experience in case management, client coordination, or customer service
✔ Background in intellectual and/or developmental disability services (preferred)
✔ Knowledge of Medicaid waivers and caregiving is a plus!
✔ Bilingual (Spanish/English) Is Preferred, But Not Required
✔ A problem-solver and strong communicator who is passionate about improving lives

What You’ll Do
- Support clients and families receiving I/DD services to navigate their care and achieve their goals
- Coordinate services, complete supervisory home visits, and attend service planning meetings
- Provide guidance on Medicaid waivers (CES, SLS, and DD) and help families understand available resources
- Collaborate with nurses to ensure health and safety needs are met in service planning
- Work closely with Direct Support Providers (DSPs), scheduling training, coordinating meetings, and ensuring quality care
- Attend community events and build relationships with agencies to strengthen support networks
