Responsibilities:
- Provide clinical social work services to patients in a Home Health and Hospice care setting
- Assess patients' mental health needs and develop treatment plans
- Collaborate with healthcare professionals to ensure comprehensive care
Requirements:
- Bachelor's degree in social work or related field
- Licensure as a Social Worker
- Weekend availability and flexibility to work Monday to Friday
Location: Cortez, CO
Rate of Pay: $29.00 - $32.00
