POSITION SUMMARY:
The Licensed Social Worker is responsible for modeling the three Company values of Compassion, Integrity, and Excellence, and for promoting the Hospice philosophy. S/he is responsible for upholding the Code of Ethical Conduct and for promoting positive working relationships within the Program. The Licensed Social Worker is a hospice-trained interdisciplinary team member who provides preventative, educational, and evaluative treatment services to meet the psychosocial needs of patients and their families, to help them improve, restore, and maintain their maximum level of coping. The Licensed Social Worker utilizes community resources and the interdisciplinary team to aid in this process. The person in this role also ensures grief counseling for the bereaved. S/he will oversee and ensure compliance of the bereavement department state and federal mandates. S/he is responsible for applying the rules and regulations of state and federal regulatory agencies and other certified agencies.
Education and/or Experience
Current, valid license as a Social Worker issued by the Louisiana Board of Social Work Examiners (LSBSWE) and a master’s degree from a school of social work accredited by the Council on Social Work Education required. One year of health care experience required; documented clinical experience appropriate to the counseling and casework of the terminally ill. Three years’ experience in grief and loss counseling and/or training. The colleague must obtain annual continuing education credits in accordance with Louisiana regulations and company policy.
Language Skills
Ability to read and analyze professional journals, technical procedures, or governmental regulations. Ability to write reports and business correspondence. Ability to effectively present information and respond to questions from groups and families.
Reasoning Ability
Ability to solve practical problems and deal with a variety of concrete variables in situations where only limited standardization exists. Ability to interpret a variety of instructions furnished in written, oral, or schedule form.
Computer Skills
To perform this job successfully, an individual should be proficient in Microsoft Office and Electronic Medical Records software.
Other Qualifications
Possess strong leadership, organizational, and interpersonal skills. Clinical experience or training related to death, dying, and bereavement. Must be of high integrity including maintenance of confidential information including all patient records. Ability to work a flexible schedule, to include some evenings and weekends. Must be able to travel frequently.
Certifications, Licenses, Registrations
Must have a valid Social Worker license issued by the LSBSWE, an active, unrestricted driver's license and auto liability insurance. LCSW PREFERRED.
