HOSPICE EXPERIENCE REQUIRED
The Intake Coordinator is responsible for managing all aspects of the patient intake process including, establishing and maintaining positive relationships with customers and referral sources, responding to customer requests and concerns, and managing the insurance verification and authorization through admission process.
ACCOUNTABILITIES
- Responsible for managing the initial intake process for new hospice patients, including gathering necessary documentation, verifying insurance benefits, scheduling initial evaluations, communicating with referral sources,and ensuring all paperwork is completed accurately to facilitate a smooth patient admission into hospice care.
- Take incoming calls from potential patients, healthcare providers, and other referral sources to assess eligibility for hospice services
- Collecting detailed medical history, insurance details, and contact information from EPIC, hospital EMRs, physician offices, clinics, etc.
- Strong Knowledge and Understanding of Hospice eligibility and Medicare Guidelines for admissions and care.
- Strong organizational abilities to manage multiple tasks and deadlines
- Ability to work effectively under pressure and in a fast-paced environment
- Benefit verification: Checking insurance coverage to confirm hospice benefits and eligibility
- Scheduling initial visits: Coordinating with the clinical team to schedule initial patient evaluations and admissions visits
- Communication with referral sources: Maintaining open communication with referring physicians, hospitals, and other healthcare providers
- Data entry: Entering patient information accurately into the electronic medical record (EMR) system
- Coordination with other teams: Collaborating with the clinical team, social workers, and other hospice staff to ensure smooth patient transitions
- Gathering medical records and documentations including HP, labs, physician consultations, social worker review of discharge planning notes from multiple sources to review for clinical eligibility
Mathematical Skills
Ability to add, subtract, multiply, and divide in all units of measure, using whole numbers, common fractions, and decimals.
Language Skills
Ability to read, analyze, and interpret general business periodicals, professional journals, technical procedures, or governmental regulations. Ability to write reports, business correspondence, and procedure manuals. Ability to effectively present information and respond to questions from groups of managers, patients, colleagues, physicians and the general public.
Reasoning Ability
Ability to define problems, collect data, establish facts, and draw valid conclusions. Ability to interpret an extensive variety of technical instructions and deal with several abstract and concrete variables.
Computer Skills
To perform this job successfully an individual should be proficiency in Microsoft Office software.
Other Qualifications
Understanding and acceptance of hospice philosophy. Must be of high integrity including maintenance of confidential information including all personnel and patient records. Ability to work a flexible schedule, to include some evenings and weekends. Must be able to travel.
Certifications, Licenses, Registrations
Must have a valid driver’s license and auto liability insurance. Active RN license.
Other Skills and Abilities
Strong leadership, organizational and interpersonal skills. Must be able to read, write and speak English.
