The ISO Support Service Worker plays a crucial role in fulfilling the mission of Passavant Memorial Homes' HCS program by providing care and supervision to the individual in the areas of activities of daily living, as well as social and recreational activities.

Duties and Responsibilities:
- Perform tasks outlined in the individual's Individual Plan (IP).
- Complete assigned tasks in the manner requested.
- Deliver services within units authorized in the IP.
- Provide supervision and positive role modeling at home and in the community.
- Complete necessary training planned by the individual/representative and facilitated by HCS.
- Observe individuals' behaviors and changes in physical condition, and report accurately.
Required Skills and Knowledge:
- Minimum of 18 years of age.
- Valid driver's license.
- Current car insurance and registration.
- Successfully pass background check.
- Experience working directly with individuals with developmental disabilities.
