Position Overview : We are seeking an organized and detail-oriented Home Health Scheduling Coordinator to join our team. This role is essential in managing and coordinating client care schedules, ensuring that our home health care services are delivered efficiently and effectively. The ideal candidate will have excellent communication skills, strong organizational abilities, and a solid understanding of the home health care industry, as well as knowledge with the Georgia Wavier Programs both Structure Family Care and Personal Support Services.
Key Responsibilities :
- Coordinate and manage client care schedules to ensure timely and efficient delivery of services.
- Schedule home health care visits for clients, including coordinating with healthcare providers, caregivers, and clients to arrange appointments.
- Maintain accurate and up-to-date records of clients schedules, changes, and cancellations in the scheduling system.
- Communicate effectively with clients, families, and healthcare professionals to confirm appointments, address scheduling issues, and provide necessary information.
- Collaborate with clinical staff to ensure that client care needs are met and that scheduling conflicts are resolved promptly.
- Monitor and track the availability of caregivers and adjust schedules as needed to ensure coverage and meet client needs.
- Handle incoming calls and inquiries related to scheduling, client care, and service availability.
- Ensure compliance with company policies, procedures, and regulatory requirements related to scheduling and client care.
- Prepare and submit reports on scheduling metrics, including client visit frequencies, caregiver utilization, and other relevant data.
