PCM continues its tremendous growth and we are looking for new team members for our national expansion. Our mission is to assist doctors in continuing communication with their patients. Our initiatives help Medicare patients who have chronic conditions work towards living a healthy lifestyle.
PCM seeks credentialed individuals to act as the liaison between patients and their providers. We hire experienced medical professionals to handle our national tele-med calls to patients. As a Care Manager, you will have an assigned group of patients to follow up with every month as their "Health Coach". Conversations include: asking about medications, symptoms, and support. As well as understanding barriers and assisting patients to reach their goals established with their doctors.
Job Skills:
Talking and typing
- Verify and record the patient’s demographics, medical issues, medications, goals, barriers and allergies to create a clinical summary using our certified software program.
Care Plan Creation
- Create a patient-centered, comprehensive plan of care for all health issues based on a physical, mental, cognitive, psychosocial, functional, and environmental (re)assessment, and an inventory of resources.
Be a Compassionate Resource
- Help provide the patient with a means to make timely contact with health care practitioners in the practice who can address any urgent chronic care needs. Ensure continuity of care with their designated practitioner by setting and reminding of regularly scheduled visits.
Requirements:
Must be currently Certified: CNA, PCT, CMA, CCMA, or RMA.
- Ability to follow HIPAA guidelines.
- Handle potentially difficult conversations in a calm and professional manner.
- Previous F2F healthcare experience required.
- Previous phone experience preferred.
- Compensation range $18-$26 per hour with incentive.
Bilingual candidates are a plus.
