RATE: $100-120.00 PER VISIT

We seek a compassionate, exceptional Hospice Nurse to join our care plan team in Southern California. The ideal candidate will provide quality end-of-life care to patients in need. This position offers an opportunity to make a meaningful difference in the lives of individuals and their families during challenging times.
Duties:
-Assesses and evaluate the patient's condition
-Continues the plan of care and makes necessary change of care plan in collaboration with the medical director depending on the patient's status
- Provide direct patient care and instructions to the family at home, board & care, nursing home and assisted living facilities
- Administer medications and treatments as prescribed
-Orders medications to the pharmacy
- Monitor patient conditions and report changes to the healthcare team
- Collaborate with physicians and other healthcare professionals to develop individualized care plans
- Maintain high standards of compliance about documentation and patient care requirements WITHIN 2 DAYS.
- Offer emotional support and guidance to patients and their families
- Ensure comfort and dignity for patients in their final stages of life
-Promotes the hospice agency to their network
Requirements:
- Hospice experience of at least 1 year is a plus
- Valid CA RN license
-Maintains a current CPR certification
-Valid driver's license, auto insurance, reliable transportation and willingness to drive
- No previous experience in a hospice agency is encouraged to apply
- Ability to work with patients of all ages
- Proficiency in medication administration
- Strong communication and interpersonal skills
- Compassionate and empathetic approach to patient care
- Active, committed, and outgoing personality
- Knowledge of hospice care
