- Degree in Human Services or related field is preferred but not required
- Experience in LTC / Rehab setting
- PCC knowledge is a plus
- Compassion for an underserved population
- Ability to problem-solve and communicate effectively Your day to day : You'll meet with families and providers to determine the best plan of action for our residents' care.
You'll assist with assessments by interviewing residents and their families and see to it that the highest practicable physical, mental and psychosocial wellbeing of each resident is attained or maintained.
