What are the responsibilities and job description for the Family Home Provider (FHP) position at Path Forward of Kentucky?
Job Summary

If you answered yes, then we want you to join our team of Family Home Providers (FHPs) in the Bowling Green area. This opportunity is also referred to as Adult Foster Care.
**Homes with Wheelchair/Assisted Mobility Access Needed**
- Participants live in the FHP's home while FHP assists them in achieving personal goals and gaining independence
- FHPs support Participant goals like improving personal care and wellness, socialization, reading or money management and aiding access to the community through various activities (i.e. grocery shopping, bowling, hiking or going to the movies), as well as daily interaction as part of the family unit
- FHPs receive room & board in addition to semi-monthly pay ($1,800 twice a month)
- FHPs document Participant progress electronically via Oasis
- FHP will receive four days of paid training ($12.00 per hour) and free online courses
Qualifications and Skills
The ideal FHP is compassionate, flexible, and detail-oriented. Access to the internet, cell phone and data entry experience preferred. A valid driver's license with reliable transportation is a must.
