Summary Purpose for the Position
To provide intake assistance, case management and other support for tenants and their families at Wilson Commencement Park. The Family Services Case Manager at WCP plays a crucial role in supporting families in need by providing comprehensive case management services. This position aims to empower families to achieve stability and self-sufficiency.
Requirements (Education, Experience, Certification, Knowledge, Skill)
Bachelor’s Degree in Human Services, Social work or other related or a combination of education and experience, with at least 3 years’ experience in conducting case management preferably with homeless or at-risk individuals and families.
Ability to work independently
Extensive housing/case management experience and the ability to work effectively with low-income families.
Knowledge of available homeless and at -risk services in the community, and transitions to work.
Relevant experience working with persons who are mentally ill, suffer from substance use disorder, are victims of domestic violence, and/or homeless.
Interviewing and counseling skills required.
Interacts in a polite, courteous, professional manner. Must be able to successfully engage with a wide range of clients from diverse backgrounds.
Working knowledge of computers including Outlook, basic Excel, Word, Publisher, and PowerPoint required.
Position must have the ability to work independently and meet specific time lines as well as work with a diverse staff and multi-programs.
Working knowledge of rental, housing, landlord/tenancy issues preferred.
General comprehension of DSS system
Must be able to use the Homeless Management Information System (HMIS)
Strong written and verbal communication skills which demonstrate the ability to interpret, clarify, advise on program objectives and tasks
Communication skills including bilingual ability strongly desired.
Excellent organizational skills.
Position Responsibilities
This position is responsible for assessing the needs of families, developing individualized service plans, and coordinating resources to ensure families receive the support they require.
The Case Manager will work closely with various community organizations and service providers to facilitate access to essential services such as housing, healthcare, and education.
Fostering strong relationships with families and stakeholders,
Complete intake application and assessment for each family/individual and determine program eligibility for Wilson Commencement Park programs; maintain waiting list of eligible residents.
Assist clients in developing/identifying/evaluating and revising a housing action plan, milestones, and short and long-term goals.
Provide case management services as indicated.
Responsible for maintaining all required records and preparation of all required reports.
Builds working relationships to solve problem and achieve common goals.
Work effectively and cooperatively with others; sensitive to the needs of others.
Safeguards sensitive and confidential client and agency information.
Offers assistance, support, and feedback in a timely and responsive manner.
Attends trainings and seminars; actively engages in skill and knowledge development.
Work cooperatively with other agencies to ensure client’s access to needed services.
Represents agency at meetings and participates fully.
Maintains accurate files and enters HMIS data in timely and accurate manner.
Engages in problem solving: suggest ways to improve performance and be more efficient.
Insure eligibility and program compliance of participants.
Shows commitment to work and to consequences of own actions.
Applies skills in interpretation, conflict resolution, written and verbal communication.
Articulates and describes Wilson Commencement Park, and affiliates program guidelines and other available services.
Other duties, as assigned.
Working Conditions/Environment
Performs home visits and outreach that may involve some exposure to injury, hazardous materials, and/or infectious environments.
Requires frequent repetitive office work, repetitive motions on the keyboard and frequent exposure to individuals at high risk of being infected with serious illness.
Must be able to lift up to 25 pounds.
Requires flexible work hours, including evenings, and weekends.
Ability to work with diverse client populations/issues
