Lead Care Manager

Lead Care Manager- Kern, Fresno, Visalia, Santa Maria, Paso Robles

(BA preferred in one of the following fields, not required)

• Social Work or Social Welfare
• Marriage, Family and Child Counseling,
• Counseling Psychology
• Clinical Psychology
• Social Psychology
• Child Psychology
• Child Development
• Educational Psychology
• Education with emphasis in Counseling
• Or other equivalent education and experience may be considered.

Summary of Duties and Responsibilities:

• Job description is in essence to provide whole person centered care management for a caseload of children or at risk families in the community.
• Provides personalized assessment of all assigned youth.
• Creates individualized Care plans and provides on-going support services to assist the youth and caregivers in meeting the goals of care plans.
• Person serves as a liaison for child and family to connect to supportive services in the following areas: Medical, dental, vision, mental/behavioral health, educational resources, respite care, and community resources.
• Provides necessary support to youth's caregivers, coaching and administrative, to engage them in services needed.
• Must past a criminal background check and have reliable transportation.

Person must live with 40 minutes of select office.

Job Type: Full-time

Pay: $56,160.00 per year
