Position Role As a Patient Care Associate you will be compassionate and empathetic person who will assist our providers in delivering top-notch care. By ensuring each patient feels comfortable and supported, you will play a crucial role in encouraging patients to return for ongoing care and wellness.
Key Responsibilities
· Primary Case Load Management:Take primary responsibility for a designated case load of patients.
· Patient Experience:Prepare thoroughly for each patient visit by reviewing clinical charts in eCW. Warmly greet patients in the lobby and escort them to the visit room. Conduct nursing assessments by interviewing patients, and measuring weight, vital signs, and other clinical measures.
· Provider Support: Communicate pertinent clinical information to the provider and assist with TeleHealth setup if needed.
· Appointment Coordination: Ensure patients are scheduled for and attend both counseling and medical appointments.
· Clinical Tasks:Perform clinical tasks as ordered by the provider, including blood draws, centrifuge operation, urine collection for various tests, and giving injections. Maintain a safe and compliant work environment by properly disposing of biohazardous materials.
· Patient Safety and Compliance: Research and document findings in the PDMP.Educate patients on prescribed medications, including proper use and side effects. Log prescriptions into the Accountability log.
· Medication Access: Communicate with insurance companies to obtain preauthorization for medications. Submit appeals for preauthorization denials when necessary.
· Office Efficiency: Complete administrative tasks such as making phone calls, setting up patient appointments, ordering supplies, and scribing to assist providers.
Qualifications
· High School Diploma or GED required.
· Excellent organizational skills.
· Ability to treat people in a non-judgmental, kind, and friendly manner.
· Previous experience in nursing care within a primary care, substance abuse, or mental health outpatient setting preferred.:
· Current CPR or BLS certification is a plus.
