Position Role We are seeking a compassionate and dedicated Patient Care Coordinator to join our team and play a pivotal role in delivering exceptional care and support to our patients. By providing empathetic, non-judgmental care and assisting providers, you will help create a welcoming environment that encourages patients to return for continued treatment and wellness.
Key Responsibilities
· Manage a primary caseload, ensuring continuity of care for assigned patients.
· Create a supportive and empathetic environment to enhance the patient’s clinical experience by:
· Preparing for each visit by reviewing patient charts in eCW.
· Warmly greeting and escorting patients to their visit rooms.
· Conducting nursing assessments, including interviews, vital signs, weight measurements, and other relevant clinical tasks.
· Assist providers by sharing relevant patient information from assessments and setting up TeleHealth equipment when necessary.
· Monitor patient schedules, ensuring completion of both counseling and medical appointments.
· Safely perform clinical tasks such as drawing blood, collecting urine samples, pregnancy and alcohol testing, giving injections, and disposing of biohazard materials properly.
· Conduct Prescription Drug Monitoring Program (PDMP) research, educate patients on medication use and side effects, and maintain accurate prescription accountability logs.
· Facilitate medication access by working with insurance companies for preauthorizations and submitting appeals for denied claims.
· Support office efficiency by managing administrative tasks like answering calls, setting up appointments, ordering supplies, and assisting providers with scribing duties.
Qualifications
· High School Diploma or GED.
· Strong organizational skills and ability to multitask effectively.
· Compassionate and non-judgmental demeanor, with excellent interpersonal skills.
· Previous experience in nursing care within primary care, substance abuse, or mental health outpatient settings preferred.
· Current CPR or BLS certification is a plus.
