Job Summary:
We are seeking a highly skilled and compassionate Registered Nurse Case Manager to join our healthcare team. As a Registered Nurse Case Manager, you will be responsible for coordinating and managing the care of patients, ensuring they receive the appropriate medical services and resources. Your expertise in patient care, diagnostic evaluation, and discharge planning will be essential in providing comprehensive and effective healthcare services.
Duties:
- Assess patients' medical conditions and develop individualized care plans
- Collaborate with healthcare professionals to coordinate and manage patient care
- Monitor patients' progress and make necessary adjustments to care plans
- Provide education and support to patients and their families regarding their medical conditions and treatment options
- Conduct regular evaluations to ensure the quality and effectiveness of patient care
- Utilize critical thinking skills to make informed decisions regarding patient care
- Adhere to HIPAA regulations and maintain patient confidentiality at all times
- Collaborate with insurance providers to ensure appropriate coverage for medical services
- Assist with discharge planning and coordination of post-hospital care
Experience:
- Bachelor's degree in Nursing (BSN) preferred
- Current state licensure as a Registered Nurse (RN)
- Minimum of 2 years of experience in a clinical setting, preferably in Hospice, ICU or critical care
- Experience in managed care or case management is highly desirable
- Strong knowledge of physiology, and critical care practices
- Excellent communication and interpersonal skills
- Ability to work effectively in a fast-paced environment
