Job Summary
Pathway’s Goal Coordinators (GC’s) lead consumer care teams to ensure that each individual is receiving high-quality care that aligns with their personal goals. GC’s work closely with consumers, Health Coordinators, Payees, Care Coordinators, and other providers to develop an individualized “big-picture” service plan. GC’s work with consumers to develop a deeper understanding of their mental illness and crisis intervention plans.
Responsibilities and Duties
- Advocate for consumers and communicate with mental health care providers.
- Develop and implement person-centered service plans.
- Provide guidance for healthy decision-making, problem-solving, and coping skills.
- Collaborate with care teams to ensure consumers’ needs are being met.
- Teach and encourage daily living skills including: monitoring medication, daily hygiene practices, apartment maintenance, and cooking.
- Assist in coordinating appointments with mental health care providers.
- Assist in establishing peer supports and community connections.
- Rotating on-call crisis intervention duties.
- Other duties as assigned.
Qualifications
- Bachelor's degree or 3 years experience in a related field. (Required)
- Effective communication skills
- Ability to reason, problem solve and make quality professional decisions.
- Valid driver’s license (Required)
- Basic computer skills (preferred)
- CPR certification (preferred)
- Caregiving: 1 year (Preferred)

Pay: $16.00 - $19.00 per hour
