Full-Time Integrated Health Home Care Coordinator with an emphisis on adolescence
ESSENTIAL DUTIES AND RESPONSIBILITIES:
The Integrated Health Home Care Coordinator will support the Health Home in meeting the provider standards and deliver Health Home services to qualified members of the Integrated Health Home.
- Understand and monitor each IHH member’s concerns and the types of whole health resources needed for the member
- Support day to day collaboration between Health Home members, primary care providers, and community resources
- Coordinates IHH member enrollment processes
- Assess each member’s strengths, abilities, and desires
- Develops care coordination plans together with each member that encompasses goals which are specific, realistic, measurable, attainable, and timely
- Provides education-related resources and health-related information to clients
- Coordinates services, resources, and other important information with each member, and on behalf of each member when needed
- Interacts with primary care providers, behavioral health providers, and other social service providers to make needed connections for services, provide a conduit for information sharing, and to organize case conferences to bring providers together to review the care of mutual members
- Works within the team model, utilizing the expertise of the team members when appropriate for each member
- Works in the community, in member’s homes, attends meetings, and assists members as needed
- Conversing with primary care physicians, health providers, and other organizational members as needed
Other duties may be assigned.
EDUCATION and/or EXPERIENCE:
1. Possess at minimum, a BSW with an active Iowa license or a BS/BA in a related field
2. Demonstrate competency with email, internet, and word processing computer programs
Job Type: Full-time
Pay: $48,613.59 - $53,504.59 per year
